I noticed that Google offers a solution for free that supports geotargeting at DMA level: https://www.google.com/intl/en_US/dfp/info/sb/index.html

you mean DMA level like, tv audience networks? or just city targeting?

Talk about a rebirth of this post. To update people, Google DFP has been great. It allows you to even forecast your inventory for the future based on geographic filters which is really helpful.
