Just a random thought crossed my head today, what if there was a tool to glance through forums/ specific topics on forums and gather the insightful data.

For instance, companies could find out what problems are being faced by consumers and why is it that one product ain't doing so well

What you're asking for is like a combo of sentiment analysis and natural language parsing, and I'm pretty sure no one is even close to offering something robust enough to identify specific questions/problems for your brand/product/whatever.

A mention tracker is simple but a decent start at least for monitoring where your keywords are mentioned on the web.

What does a mention tracker do, is it only used to check social signals online?

Just did that with this thread (minus my post)

I went through that site, and it's still quite naive, not as robust NLP as possible.

Probably a lot faster and easier to go to Yahoo Answers and other sites like that to see what problems / questions consumers are having ;)

Yea, but I think what the OP wants is some automation to do that, rather than visiting all sites on the net to find out the questions/problems that some might have.

I'm surprised though that a place like Google hasn't done anything about this so far
