Billing on Value

Just read a Shoemoney article where he mentioned billing a dentist based on the customer's value. I like this concept, seems to work well all around and i think I could implement this.

But how do you compute how much the customer is worth? Do you guys have any suggestions? Shoemoney suggests allowing the dentist figure out the customer's value but I think that's a double edged sword.
 


It just gives another option to those offline businesses that are uncertain about the value of allowing you to advertise for them. If their perceived value of each customer is too low, and it's not worth it on an incentive basis, than you can simply charge them a flat fee for your services.

It's all about giving the offline business, options, along with yourself, to clinch a deal, especially when it comes to offline businesses not seeing the benefits straight up.
