Design help needed with Wordpress registration plugin

I have a customized plugin for WordpressMU for user registration. A few screens where users enter their business info, etc.

Need someone with design skills and a wordpress server who can make the forms and pages look good. Obviously it is the user registration funnel so needs to be good, right now it works but looks like hell.

You would need to install the plugin on your own Wordpress server and fix the look.

Please let me know costs and see some samples of your recent work, especially any registration forms etc.
