A few weeks back I asked for help after getting banned. I have been up and running for 3 weeks now.
New IP - even if you have a dynamic IP it doesn't reset all the time even if you switch your router off test (myiptest.com) the easiest way was to call my provider and they just reset it
New CC: Just call CC provider say it was lost new one will have different numbers
New Name: as suggested I tested a fake name with the CC and it works all you need to do is use correct CC number street number and correct zip code (all numbers) everything else can be changed
New PC: Got a lot of instruction on how to delete cookies clear other information the easiest way was virtualbox.org it is free got the advice from someone here. Just create new PC when needed without having to worry about deleting stuff or buying new PC.
Don't start off with 1k+ daily budget start small
