So every day, I get an average of like 10 or so leads...

I consider "leads" that land at
DesignCourse.com | Online Web Design Courses then click the blue order button (which clearly states the price of $49.95 next to it) and fills out the form located at:
The Premium Online Design Course | Learn Web Design and Logo Design

So 10 people per day, who know the price, click the blue button and fill out the form and click "Continue to Final Step"

That button takes them to my order form, located here:
https://secure.designcourse.com/join

The conversion ratio is less than 1 per day, it's about 1:20 (lead/join ratio).

My question is, are there any noticeable issues with the order page that may deter people from joining?

You have a lot of scary emphasis on cancelling, yellow freaks me out. I would highlight the savings ("Seriously, just $49 bucks a month!") and put less emphasis on the possible negatives.

On a related side note, are you dropping those half submitted forms into a follow up auto responder series? you could probably bump conversions by staying on their asses a bit.

you need a better headline on the home page.

some crayz shit like

"How I make an Extra $2,430 a month designing logos in my spare time"

shit like that. your current headline is pretty lame

"To ensure DesignCourse.com is able to provide each member with the highest quality training, we charge a monthly subscription of $49.95. There’s no risk involved, we provide a 30 day money-back guarantee."

"DesignCourse.com is just $49.95 monthly and includes the highest quality design training available. We're so confident you'll love it that we proudly offer you our risk-free 30 day money-back guarantee!

<no yellow highlighting anywhere on page>

"Proceed through the form below and gain membership access within minutes."

"Just fill out the secure form below to gain immediate access to over (100) <some number> tools/pages of info/lessons"

"Your Membership"

"Your Membership Details"
-center the submit button
on the right you have your quote- ....the first 30 daysyou " - you need a space between days and you.

Personally I'd change to the bolded text

I'd also add an exit pop for those closing the page that redirects to a slightly different verison of this page with some 'perk'

1 month free, 500 psd graphics in a file you'll email, whatever.

Sell via friendly styled writing (not WAIT WAIT WAIT WAIT! You're GONNA MISS OUT!" - but instead something like "Before you leave we'd like to offer you a special offer. We're really passionate about DesignCourse.com and know that you'll love it. Give us a shot, again you have nothing to lose with our 30 day guarantee. And to say 'thanks' we'll also offer you _____________. - Click the cancel button below to take advantage of this offer and give us a shot!")

<okay to continue cancel to stay on page>

It might not be a bad idea to make Paypal the default either.
It's safe to assume that most of your visitors have a paypal account, maybe I'm wrong.

Being asked to fill out your billing address and all the bullshit will definitely turn some people away out of pure laziness.

What are they clicking on that page? Your navigation, the refund policy, the back button? I would run clickheat or crazyegg on it to see what they're doing instead of filling out the form and clicking the button.

Lose the radio buttons with Credit card or paypal, just use the paypal logo button somewhere around the credit card form. Asking the lead to choose just adds friction.

Also, record the on focus and on blur of the form fields, see if people are jumping off at one particular step.

Lose the navigation. All of it.

Move your satisfaction guarantee below the testimonials, directly across from the CC number field. Move your SSL thing right there too.

Put an explanation next to the CVN (or card security code, or CVV or ... no one has settled on a name for that bit, and people still need you to tell them where it is on their card. Related: what's with the parentheses next to the acronym?

Focus your cancellation / refund / 30 day trial that's not free but can be message. Just put a pre-checked checkbox next to this: Join now for only $49.95 with our 30 day money back guarantee. (or something) The point is - just say it once in clear, honest language. Having links to "details" just makes me think something shady is being hidden.

Put faces next to the testimonials. Smiling confident faces looking non-threateningly into our leads eyes.

Change "Submit Order" to "Finish Signing Up" or "Start your subscription"

If the number starts with a 4 it's a visa, 5 is mastercard.... why make the user specify at all? If you want to indicate which cards are accepted, move their icons to that box.

put month names after their numbers in the expiration date bit.

Put your phone number closer to the CC field.

"We will not share your email address with anyone" (or appropriate for your circumstance. Or, "Your email address is your username" or etc, give them a reason why you want it and they'll assume that it's safe to give it to you.

Only Click Once - that's javascript or your databases job. no distractions or veiled threats near the Action Button

This sounds like you're embarrassed to be charging so much:
To ensure DesignCourse.com is able to provide each member with the highest quality training, we charge a monthly subscription of $49.95.

Don't make excuses for charging money. It has a cost because it has value - that's all you need to say.

"I agree to the terms of service and the refund policy as stated here" - reverse the placement of terms and refund - i was curious about the refund policy, so I clicked it. Now I'm in a new window and hey look, a squirrel!
People are easily distracted, cut out every step you can and don't give them any other options. If they want to read more, that's why god made the logo and the back button.

put an example of the format you want for your credit card field - even if you are going to strip it all to numbers anyway (and you really should). Let them type it however they want, but give them the hint so they don't have to think to hard about it.

Labels on top of field elements. It makes filling out a form much easier. If you insist on putting them on the left, put them right next to the input box and add some margin to the top and bottom of the row.

Complete your subscription
-- test "Finish your order" or something that doesn't say "subscription" and communicates that they're getting something now

One more thing - for being a course about design - there's not many pictures. Screenshots, man, lots of them - everywhere, like goddam png confetti. Logos and swooshy widdly scuds galore.

I'm going to go outside and criticize my neighbors dog.

Jack, I'm going to implement what you said and report back shortly.. Thanks a bunch everyone.

Several good points above.

Following based on your new design

+1 for not requiring the card type for the cc. If you are using the Paypal IPN, the card type field is optional afaik

Replace the default IPN errors with friendly errors:

e.g. this:
50.0 The PayPal API has returned an error! Ack: Failure Correlation ID: bb042618498c4 Version: Error Number: 10527 Short Message: Invalid Data Long Message: This transaction cannot be processed. Please enter a valid credit card number and type. Error Number: 10508 Short Message: Invalid Data Long Message: This transaction cannot be processed. Please enter a valid credit card expiration date.
with

Invalid card number, please check you have typed it correctly
or at a minimum, just the Long Message from the API

The complex, technical, red message may scare some users off or suggest an error on the site.

I assume the card number field will take the number without the dashes.
I would parse the input to allow:
0000-0000-0000-0000
0000 0000 0000 0000
0000000000000000

However, i would change the comment below to:
Example: 0000000000000000
or even
Example: 4111111111111111

so the user does not feel they have to insert dashes.

CVV pic beside the field

Replace "The 3 digit number on the back of your card" with t"he 3 or 4 digit number on the back of your card." as Amex is 4 digits or change this via js based on card number/type

Check your visitor logs and put and other popular countries (e.g. UK, European countries) at top of county list w/US and Canada or even prepopulate country field based on GEOIP lookup

Pressing enter on any form element triggers the Paypal form Submit, not the "Finish Your Order" form submit.

It would be a bonus to highlight the form element casuing the error in pink/yellow and scrolling to the element rather than putting them at the top.

Make it more obvious there is a choice between Paypal and entering CC details directly.

Don't make ZIP an obligatory field for countries without a zip code system - see below for cc list of those countries:

PHP:
"al,at,sn,ae,af,ag,ai,an,ao,aw,bb,be,bf,bi,bj,bn,bo,bs,bt,bw,bz,cd,cf,cg,ci,ck,cl,cm,co,cu,cv,dj,dm,do,ec,eh,er,et,fi,fj,fk,ga,gd,gh,gi,gm,gn,gq,gs,gy,ie,io,iq,jm,kh,ki,km,kn,kp,kw,ky,lb,lc,lr,ls,ly,ml,mm,mr,ms,mu.mw,na,ne,ng,ni,nl,nr,nu,pa,pe,pi,pn,qa,rw,sa,sb,sc,sh,sl,so,st,sy,tc,td,tg,tk,to,tt,tv,tz,vc,vg,vn,vu,ws,xd,xe,xx,ye,zw,ug,hk,tf,sj,aq,bv,hm,mo,sr,ua,kr,bd,in,mv,np,pk,lk,mn,ir,id,la,my,pg,ph,sg,th,tl,tr,bh,om,il,jo,cy,eg,am,az,ge,kz,uz,tm,tj,kg,dz,ma,sd,tn,er,so,ps";
I would put the phone number right at the top and indicate it is possible to pay by phone also.

I would also increase the contrast between phone number and detail greys and background greys.

Autopopulate state field with none or grey out if non-US country is selected.

Put an onclick event on "I agree to the terms of service above." to tick the box so the user does not have to click directly on the tickbox.

Didn't read the rest. Sorry if was already mentioned. My 2 cents:

1) Convert drop downs to radio buttons. Easier to visualize all the choices.
2) Remove unnecessary inputs. Do you really need age and sex? I don't think you do. At least don't ask for these on this step.
3) Do not put so much emphasis on "Your information will not be shared".
4) Rename button "Continue to final step" to something that doesn't indicate that there is another step. It's a turn off to know that there is another step.
5) Change your blue title to something more interesting and call-to-action-y. Now you are stating facts. Boooring.
6) Remove thank you notice, it's just making things look more busy than they need to.
7) Make your form inputs larger, like 2x larger. Make it look big and important.

hi bud, this thread is worth its weight in gold. One thing that might also help is some kind of mouse tracking like clicktale for example. Be able to watch vids of your leads navigating their way though.
