Need a coder who can setup web form and data collection

I need someone who can write out the form coding and everything I need to start collecting data from my website forms. I need the data to be able to be saved to a database or a file to be read in a csv. Just need an easy way to collect web form information and then retrieve it easily so i can sell it.

This is probably easy stuff for a coder so pm me if you can do it and your price.
