Wanted: someone to tweak my php sendmail script (quick)

I have a php sendmail script (regular e-commerce form: name, address, mail, number, click to order) and I need someone to

- implement a dropdown selection
- make the form work (collect data, send mail to customer)

the current code I have works - except for the dropdown...I sincerely dont know how to implement it.

So...if someone can take a look at my current code and install a nice dropdown and make it work...fire away please
