Web Form Help

I'm building a site that has many pages with lead forms that go to different advertisers. I want to be able to have a form sent directly to the advertiser and cc'd to me as well. Also, the forms each need to have a unique title or tag so I can tell which advertiser they are meant for.

Does anyone know how I should approach this? Is there some out of the box solution I could use? I'm building the site in wordpress and I'm not much of a coder.

if your doing the actual php for the site, it shouldnt be hard at all, just add into every from another action that sends you an email.

I'm actually really bad a programming so I like to use wordpress to help. The entire site runs on wordpress. I've tried using some third party solutions but they all only send leads to one address. I want to be able to send leads to the advertiser (100 different addresses or more) and cc it to me. Any ideas? Sorry if there's some newb solution im overlooking.
