When Requesting a LP, what to specify?

Ok, so in general terms, I have requested a Landing Page from my AM on two occasions.
The first time it was a general request, he responded back "let me know the specs for what you want and I'll submit it to design."

Ok, so I had a new idea, I asked for an LP again, and this time I specified what I wanted. I sent the AM this.

) I'd like to request a landing page from the design team if thats ok. Specs would be
- 1 page
- Header question "What are the xxxxxx?"
- Call to action " tell us and get a <whatever the offer is>"
- Poll choices (with images or logos, radial selector)
- choice1 (choices hidden)
- choice2
- choice3
- choice4
- choice5
- then the iframed email submit, matching colours with page.

Is this acceptable for an LP request? Is there some thing I have left out? Any suggesting when requesting LP?

My AM never responded and its been a few days. I know I should follow up, but I want to be sure I am telling them the right things when they ask for specs.
Also, when I requested it I mentioned it was time sensitive so I feel the prime time for event I wanted promote has passed... so, any tips for next time?
 


If I got that specification from a client I would not really know what to do with it.

Firstly, right a small paragraph outlining the purpose for the page, target audience and any general information you think will be applicable (I usually expect color scheme etc. or a few links to examples).

Also - your specification layout makes it quite hard to understand, a simple word document with a few tables to give a visual guide is great.

Its really for your own benefit because you will be getting a landing page that fits what you need, instead of a broad match.
