That design is your main problem. It's too vertically oriented and the buttons at the bottom are too ambiguous as to their purpose. Try using a more landscape-oriented format. Also, users generally don't like video or sound playing without their control. This will get a lot of people to click back instantly.. unless it's small and concise, like "get a free iphone!" Generic techno music, though...

Put the zip offer in an Iframe and cover up everythign but the actual form and submit button with CSSthen put an overlay that says "Free porn/downloads/music/etc fill in your zip/email"watch the $$$$$$ flow in
