WordPress Popup and Lead Capturing Plugin | Pippity Popups
The OptinMonster guy is the same guy who does the Solilloquy slider, which I've been incredibly happy with both support and feature enhancement, so I went with OptinMonster instead of Pippity. Super easy to use, well thought out features, not as much customization as I hope is allowed in future version. A bit expensive, but I had a client buy me a dev license, so that made it justifyable.
