Lead Generation Fraud Prevention

For those of you running your own lead generation campaigns (you are the offer/advertiser), what methods/techniques/services do you utilize to reduce fraud and improve lead quality?

I've used TargusInfo in the past with good results. My only caveat with them is the number of false positives.

Ban affiliates from high fraud risk countries.
Ban duplicate IPs from submitting differing lead information.
Ban public proxy IPs from accessing lead gen page.

Are you doing this in practice? Are you using some kind of DNS Blacklist? The first thing that comes to mind is the Spamhaus XBL (The Spamhaus Project - XBL).

Cross-reference everything. Make sure Area Codes match with Zip Codes and States. Make sure email domains exist.
