I am currently look for a graphic design and a web designer to construct a very simple but professionally developed site. Below is What I am needing.

One single page with a nice large image focusing around bars/restaurant / exclusivity.

On that page I want a simple form for visitors to submit their name and email address. Then forward the information to an email.

Very simple but I would like it to look extremely professional.

Please responsd with portfolio, estimated cost and estimated time.
