Please read my requirements carefully.
I've got a website that consists of 3 php pages. Each page has to be redesigned according to a JPG image a designer created for me. The functionality will not change, only the GUI.
Job description:
1. Redesign the 3 pages according to JPG image. Price $55
2. Code a new php page that provide data entry into a mysql table. Price $18
The requirements are the following:

This is the website with 3 pages:
Page 1 (index): Islamic Videos
Page 2 (Category expanded): Islamic Videos - Dr. Zakir Naik
Page 3 (view video): Islamic Videos - Is the Quran God Word - Part 3
You will notice that the jpg is only for index, so the developer will have to improvise for page 2 + 3 that fits the design. Therefore the developer should also have designer skills.
The developer has to deliver a draft (for review) every 3 days max. until the job is complete. He/She should expect 1-3 reviews per page.
The code will be for php 4.3.9 and must validate and work on all major browsers. The php pages I provide must be customized instead of recreating new pages, to ensure retaining all the seo work (header tags, title tags, parameters, etc).
The table width and height should not be fixed, it should adjust to the users screen resolution (as it is now) with a minimum resolution of 1024x768.
Payment is $55 via paypal after delivery of finished work. The next task is to create a new php page that can provide CRUD functionality to a single table, which is $18, more on this later.
