Anyone know of any mortgage companies that offer seamless on site forms to their affiliates? I know CJ has a few advertisers offering interactive banners, but nothing that is really seamless.... or does something like this even exist?

yes it's called a white label and there's a few ways they work:
1. iframe their forms in your page
2. host a form on your page and post the data to that server from your form
3. (the best way) host the form, log info to a db, have a script send your data to their server
Depending on your amount of traffic will determine the options you have available to you. Contact me on IM, smaxor on yahoo works best. And I'll let you know some places.
