Pay Per Call Campaigns? How do they work? [Like magnets?]

Can anyone please give me a little rundown on how pay per call campaigns work. I've been seeing that many PPC campaigns are the highest EPC campaigns on a few networks and I've yet to actually try it.

I'm primarily a CPA affiliate that uses display and social and have been pretty successful since about 2011.

What type of traffic sources are people using for Pay Per Call?

How do the landers look if you're driving someone to a phone number above all else?

Are people pairing those Pay Per Call phone numbers with lead gen offers?

From what I understand, here's an example: customer goes to google search on their mobile device. If they click on an adwords ad, it gives them an option "you are about to call number 1-800-xxx-xxx". Or, another option is if they click onto your lander, in which there's a phone number on the LP to call. If customer calls and stays on phone call for x amount time, then the affiliate gets compensated.
 
They have high epcs b/c close rates on the phone are much better than online, hence they can afford to pay whatever.

Flip side is your cpcs can be extremely high and while other traffic sources can get you some competitive traffic, it is a challenge to both make it work on your end & the advertisers'
