The zombie in the room ate Lori. That's why the beer belly. It's all of Lori inside of him.
And yeah, he is talking to himself on the phone. He's a little crazy. When Hershel picked up the phone, he didn't hear a dial tone or anything.
They didnt attack her because she was covered in zombie guts and therefore smelled like a zombie to them.
