Since there's nothing I can to fuck with him, I just blocked the referrer in .htaccess so I don't have to keep watching clicks to that page coming in.

This is simply not true. You need to think outside the box. There are many ways to fuck with this guy.

Back tracking the rest of his business should be pretty easy with someone this dumb.
You can always fuck with the advertiser and or network

Is there not something you can change in the pixel itself, like say, instead of the actual 202 pixel, change the code to redirect the pixel fire to a bad neighbourhood so all AV blocks the traffic to his site?
