I've been thrilled w/ westhost.com.

Though it's not cheap, and if you're doing anything real sketchy (porn, etc) avoid it. Service cannot be beat however.
 
I've been with loads of dedicated places.

I hated softlayer tbh because while they offer great deals, their support (at least when I was there) was fairly terrible.

Hivelocity was better but kept screwing up (4 downtimes on their fault - costing me thousands and never got a cent or even a free month).

FDCServers... sucks.

LayeredTech again support was meh when I was there.

Liquidweb has by far had the most amazing support. I have 5 servers with them and support times take usually around 10 minutes max... most of the time faster.

I also love the option they have of using their DNS servers meaning you don't have to setup your own private dns servers (which is better for affiliate marketing).

I am software engineer, and I used 20+ web hosting companies

My best hosting companies are:
1) Linode
2) Slicehost

I use Ruby on Rails, they support it well. They are cheaper than most web hosting companies the other guys listed and they offer real quality.

$170 is not pricey. I pay $250.

If you can admin your own box, they are hands down the best option.

They have support for $3 a ticket now. As long as you have monitoring, you shouldn't need dedicated support unless you are hosting IMO.

I'm a fairly new customer with them (2 months) but their support has been kickass. For a $3 ticket, they will work their butt off to take care of an issue. Super polite, fairly prompt... I've worked with management firms on top of hosting (LT) and the service is always wanting. Ticket times start to go from 1 hour, to 3 hours, to 6 hours.

Now maybe I'm just lucky, but SL has been heaven for me, and I've been with LT, Dedicated Now, and a bunch of shared and VPS hosts in the past.
 
My votes would be for LayeredTech (I've had a server there for probably 5 years now) and InterServer. InterServer provides semi-managed servers (https://interserver.net/technical-support-overview.html) at great prices, with a great network - If a special package listed doesn't work for you, give them a call and see what they can customize. You can customize using their online form, but you'll probably get better info and potentially better pricing for customization over the phone.

Interserver has had the best support out of any server company I've ever been with (and I've had servers at 10+ providers since 2002, and used to sell dedicated servers too)
 
Softlayer had way too much downtime and data center problems when I was there. I moved to theplanet.com and haven't looked back since. Gotta a fairly powerful box with Raid 1 and additional SAN backup = $306.20 every month and worth every penny. Also using BQ Internet for offsite backups via rsync = $10 per month.
 
I use both Softlayer and LiquidWeb and they're great. LW especially has ultra-fast, friendly and truly helpful 24x7 support if you ever need help with anything. Softlayer is great too. They may cost a little more than many competitors, but it's worth it.
 
Go Mosso! Rackspace owned cluster/cloud hosting. Had some problems in the past but stuff has been great recently. Great stuff not having to worry about servers exceeding capacity.
 
Hurricane Electric was the best! but no longer offers shared. I have my own servers lots of them and they are always a pain in the ASS - but we have no choice. For your needs if you are going with shared. I too would suggest Rackspace or Liquidweb both top of the line.
