The odds of anybody in their social circles seeing that is pretty limited. And if so then they should ask their social circles what in the heck are you doing on 4chan? It's one of those things that the people that out them have to be into some F-ed up stuff just like them to find it.

Yeah, but it looks like somebody clearly made this with them in mind. Unless it was just a random dude who noticed the similarities, I'd bet the creator of that graphic doesn't like one of those people and will try to ruin their lives.
Bro 4chan will track down where they live and their facebook and other social networking accounts and will mail it to their neighbours.
And then some dude who lives in the area will knock on their door and be like "you seen this shit?" They don't really stop until the whole family is ruined
